Please take a moment to read the survey rules before proceeding to the actual survey. You may click on the Survey Link Button at the bottom of this page to begin the survey. There is no time limit for the survey but it must be completed in one sitting. Once submitted, surveys cannot be changed. The survey takes about 15 minutes to complete.
Survey Rules
All information gathered in the survey will be held in strict confidence and will not be shared with third parties or outside entities. We are gathering this data for internal use only, to be used exclusively to improve our service.
Each completed survey will be entered into a draw. Draw winners will receive either a cash payout (in the form of a mailed check) or credit on their PharmStore.com account to be used against future PharmStore.com purchases. 15 names in total will be drawn – 12 primary winners and three alternates. In case we are not able to contact the primary winners (three attempts in total will be made via phone and/or email), the alternates, in the order they are drawn, will be declared as winners.
We ask that survey-takers answer as many questions as possible but only the contact information questions are mandatory (marked with an Asterix*) – we want to make sure draw winners get their prizes! If you’re not sure about any answers, please leave the question blank. Surveys with only the contact information filled out will not qualify for the draw. Once the survey is completed and submitted, answers cannot be changed. Answers may be changed before the survey is submitted.
No purchase is necessary to complete the survey or to qualify for the draw prizes, but participants must be either current or past Pharmstore.com customers to participate (have ordered in the last six months from PharmStore.com).
The survey will run for approximately six weeks starting on October 1, 2024. The survey period may be extended as needed but will not exceed eight weeks in total. The prize draw will occur within two weeks of the close of the survey.
The survey will be available as an online form, or survey takers may call PharmStore.com at 1-800-281-8347 to complete the survey over the phone with an agent.
Each PharmStore.com customer will be limited to one survey/draw entry but entries are not limited to households. For example, if more than one PharmStore.com customer resides in a single household, both may complete a survey and both qualify for the draw. If the second person is unable to open the survey (if the message, “You have already taken this survey” pops up) then please call us at 1-800-281-8347 and an agent will take the survey with you over the phone.

The total prize pool is $1,275.00 in US funds.
Draw winner names will be checked against our customer list and non-customers (those who have not made a purchase with PharmStore.com in the last six months) will be disqualified.
Prize winners will be notified by email or phone, so please ensure your survey contact information is correct.
We will post the winners’ names, using their first name and last name first initial, as well as the state they reside in on our website. When we contact the winners, they may decline to have their names posted on the website.
Each survey will be assigned a number and a random number generator will be used to randomly select draw winners.
